Technical Assistance:

Technical assistance was provided regarding new resident agreements and criminal background checks for staff.

Comments:

Type of inspection: Initial Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 11-22-22 from 10:00 a.m.- 11:00 a.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Additional Comments/Discussion: An initial inspection was conducted by licensing staff due to a change of ownership. The owner is requesting that the two separate facilities on the same property be licensed as one facility. In addition to observing the physical plant, facility documentation and facility postings were also reviewed for both buildings. A conditional license will be recommended. An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
