Type of inspection: Complaint
An unannounced complaint inspection was conducted on-site on 11-18-22 (ar 090:30/dep 2:30 p.m.) The facility census was 77. Day 2 on-site conducted on 12-12-22. The administrator was not present during either on-site visit. The assistant administrator signed and dated the Acknowledgement form following an exit meeting on both days.
The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.
A complaint was received by VDSS Division of Licensing on 11-16-22 regarding allegations in the area of buildings and grounds.

The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility.

An exit meeting will be conducted to review the inspection findings.

The evidence gathered during the investigation supported the allegation of non-compliance with standard(s) or law, and violation(s) were issued. Any violation(s) not related to the complaint but identified during the course of the investigation can also be found on the violation notice. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Based on staff interviewed, the facility failed to ensure it reported to the licensing office within 24 hours any major incident that has negatively affected or threatens the life, health, safety, or welfare of the resident.

Evidence:

1. The licensing department received a complaint on 11-16-22 alleging ?since September of 2022 the memory care unit has been infested with bed bugs there crawling on residents and they don?t? know what?s biting them, they?re in the furniture the residents sit on...?
2. On 11-18-22 during Interview with staff #4, staff stated there was a couch that been identified as having bedbugs and this piece of furniture was thrown out. The facility contacted its pest control provider and rooms were treated and the dog also came to assist with locating the bedbugs.
3. On 12-20-22, during third exit meeting, staff #1 acknowledged it did not notify the licensing department of the bedbug in the facility.

Complaint related: Yes
Description: Based on documents reviewed and staff interviewed, the facility failed to ensure the building was kept free of infestations of insects and vermin.

Evidence:

1. On 11-18-22 during a complaint inspection, staff #4 provided the inspector with the facility?s ?Special Service Request? document that noted the facility was treated for bedbugs on 10-13-22, 10-19-22, and 11-3-22. The inspector was also provided an invoice from a pest control company, dated 11-14-22, documenting the facility?s inspection for bed bugs.
2. On 11-18-22, staff #3 and #4 acknowledged the facility did have an issue with bedbugs and the issue was contained to one area of the facility.
3. On 12-20-22, during third exit meeting, staff #1 acknowledged the facility did have bed bugs.
