A self-report from the facility was received regarding resident care, and violations were found at the time of inspection.

Violations:
Standard #: 22VAC40-73-40-A
Description: Based on resident record review and staff interview, it was determined that the facility staff failed to follow the facility's policies and procedures.
Evidence: On 1/1/2023 Staff A and Staff B entered Resident A's apartment after hearing her voice. Staff A and B found Resident A on the floor and bleeding from her arm. Both Staff A and B left the apartment to get Staff C who was on another floor. Staff A or B did not stay to render First Aid to Resident A nor did Staff A or B call 911 as the facility policy and procedure states.

Plan of Correction: Within thirty days, CPR and First Aid certified staff will participate in a medical emergency drill concerning First Aid and CPR. Each of these associates will sign an affirmation stating they will comply with Commonwealth Senior Living's policy on First Aid, CPR and Staffing as with standard 40A. Current associates will receive a refresher on First Aid and CPR.

Standard #: 22VAC40-73-450-C
Description: Based on resident record review and staff interview, it was determined that the facility failed to develop an Individualized Service Plan that identified all of the resident's needs
Evidence: Resident A's Individualized Service Plan did not include her need for off-site hemodialysis.

Plan of Correction: Within thirty days, the executive director and resident care director will audit each resident's record to confirm that each resident's care needs are reflected in their Individualized Service Plan. The executive director and the resident care director will confirm that updated Individualized Service Plans are accessible to appropriate staff members at all times.
