The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility.

Observations by licensing inspector: This program has had two previous walk-through inspections. Room sizes meet all the standard requirements for the capacity requested. The facility would like to start with nine moving eventually to 12 with added space. This inspection was completed virtually to have a current review. Nothing has changed since the walk-through inspections. Due to Covid at the time of the initial review opening plans were placed on hold and the previous application withdrawn. Covid interfered with not only staffing but all attempts to get the fire and safety inspections which have now been completed. A new application was submitted. The program will be residential only with the target population being those individuals who meet the criteria to live more independently following participation in a transitional training program. Aside from typical activities as provided on the schedule, staff will be working with residents on meal preparation, approved by the health department as per discussion with staff and this licensing inspector, moving toward self-administration of medication, utilizing community transportation and other independent living type skills. This would be the next phase from the initial transition training received prior to moving into the new location.
Additional Comments/Discussion: The budget information provided is based on the initial plan for twelve individuals as the capacity will likely change in the next month. The owner wanted to be sure that the change in capacity would be covered as well in the calculations as he had hoped it would be completed at the time of licensing. Since the program is residential only it does not require a licensed administrator. The named administrator, however, has supervisory experience, experience in working with persons identified as having mental health issues and has completed additional training related to overseeing the program.
Fire Inspection: 3/28/2023
Health Inspection: 4/12/23
The administrator is aware they will be issued a conditional license which will expire six months from the date of issue. At that time a renewal application will need to be submitted but no additional funds will be required. The program will have interim inspections during that six-month period. The results of those inspections will determine the outcome along with the renewal inspection results. There is no risk assessment profile for the facility at this time as they are brand new.
An exit meeting will be conducted to review the inspection findings. No issues identified at this time.
