Documentation was discussed with the provider.

Comments:
An unannounced renewal inspection was conducted on 5/10/23. At the time of entrance, 206 residents were in care. Meals, medication administration, and activities were observed. Building and grounds were inspected. Records were reviewed. The sample size consisted of 10 resident records and four staff records. The violation was discussed and an exit meeting was held.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (I) type the plan on a separate Word document, (II) identify the standard violation number being addressed, (III) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Violations:
Standard #: 22VAC40-73-260-A
Description: Based on record review, the facility failed to ensure that each direct care staff member maintains current certification in first aid from the American Red Cross, American Heart Association, National Safety Council, American Safety and Health Institute, community college, hospital, volunteer rescue squad, or fire department. The certification must either be in adult first aid or include adult first aid.
Evidence: No documentation was provided, during the inspection, to confirm that Staff #2 (hired 8/9/22) and Staff #3 (hired 11/15/22) have current first aid certification. The records of Staff #2 and Staff #3 contained current certification for CPR and AED, but not first aid.

Plan of Correction: All RMA/CNA's will have updated CPR and First Aid completed by September 1, 2023. All files will be audited to ensure that each team member has valid certifications. Team members can attain CPR/First Aid with our current in-house BLS instructor or an alternative valid instructor.
