Census 56 Number of records reviewed and interviews conducted- 1 record, 4 interviews. All facility self-reported incidents since the last inspection were reviewed on this date. There was an allegation of faulty equipment and inadequate staffing to meet the needs of a resident in care. Part of the allegation was determined to be valid. A violation was issued for this inspection.

Complaint related:	Yes
Description:	Based on resident record review and staff interview, it was determined that the facility did not have equipment in good repair at the time of the inspection. Evidence: Resident A had a pendant that was not in good working order at the time of admission on March 2, 2023.
Plan of Correction:	The pendant for Resident A was changed as soon as it was discovered. Staff monitored this resident all through the late day and night upon admission. All pendants throughout the Branch will be checked to see if in working order.
