Type of inspection: Complaint
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 5/24/23
The Acknowledgement of Inspection form was signed and left at the facility on the date of the inspection.
A complaint was received by VDSS Division of Licensing on 5/22/23 regarding allegations in the area(s) of buildings and grounds (urine odor in common areas) and possible danger pet in building.
Number of residents present at the facility at the beginning of the inspection: 27
The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Staff admitted there had previously been two problems relating to an odor in the common areas. One was directly related to an issue with the kitchen drain in which the previous people who clean the drain broke the snake off. Over time there was building up, an odor and problems with drainage. That has since been repaired and the drain is also being moved to another location in the room. During the time in question was an issue staff was having with a resident on visitation from the state hospital who refused to allow them to assist with toileting. The individual was extremely incontinent and due to his medication, the urine had a very strong odor. The same was true for a resident at the facility that staff is diligently working with to allow their assistance. Both were in the common area when the pet therapy dog was visiting. This inspector interviewed the available resident in question, and it was clear that staff?s report was accurate.

Observations by licensing inspector: No odor in common areas at time of inspection. Facility has ordered new flooring as the current flooring allows moisture to gather underneath resulting in loose tiles as well as potential odors.
Additional Comments/Discussion: The dog in question is a trained therapy dog and has been visiting the facility for several years. The facility had paperwork related to both the training and vaccinations for the dog. No one interviewed was aware of any incidents during the weekend visit. The administrator said in the future she would schedule a time for the dog and have it in an area that only those residents interested would be assisted to participate. None of the residents indicated a problem during the other portion of this inspection process.
An exit meeting will be conducted to review the inspection findings. The evidence gathered during the investigation did not support the allegation of non-compliance with standard(s) or law.
