A complaint was received by VDSS Division of Licensing on 7/25/23 regarding allegations in the areas of: Administration and Administrative Services, Accommodations and Related Provisions, Building and Grounds. Number of residents present at the facility at the beginning of the inspection: 39 Number of staff records reviewed: 0 Number of interviews conducted with residents: 1 Number of interviews conducted with staff: 1 Observations by licensing inspector: Building and grounds, resident rooms, facility maintenance and repair/cleanliness Comments: Inspected systems put in place to prevent elopement of resident. An exit meeting was conducted to review the inspection findings.
