Date of Inspection: October 16, 2022 Type of Inspection: Monitoring inspection Census 7 Number of records reviewed and interviews conducted- 6 records, 4 interviews. All facility self-reported incidents since the last inspection were reviewed on this date. The Licensing Inspector observed residents pursuing independent activity pursuits, eating lunch and preparing to go on an outing. This Licensing Inspector also observed medication administration and compared Physician Orders to the medications available for the residents. Violations:

Standard #:	22VAC40-73-320-A
Description:	Based on resident record review and staff interview, it was determined that facility staff failed to obtain a physical examination and report within 30 days preceding admission. Evidence: Resident A's physical examination and report was signed by the physician on 7/11/2023 and the resident was admitted on 9/22/2023.
Plan of Correction:	Beginning 11/3/2023 staff will ensure that a new Health and Physical examination is obtained within 30 days prior to a new admission to the facility.
