Your time and assistance were appreciated during the unannounced monitoring inspection conducted on May 15, 2018, between the hours of approximately 10:00 a.m. and 2:50 p.m. Forty (40) children were enrolled and thirty-eight (38) children were in the care of six staff members during the on site inspection. One additional staff member was present and assisted, as needed. Five children's records and five staff records were reviewed and/or updated. Three children's medications and two children's topical ointments, as well as authorization forms and administration logs were reviewed. Program activities, daily routines, interactions with children, staffing, supervision, equipment, posted information, agency inspection reports, verification of insurance, emergency supplies and drill records, snack/menu, the classrooms and the outdoor playground areas were observed/reviewed. Lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage program specific learning and play materials. Program observation was completed. Activities observed on this date included: children's work cycle; group time in the 2 and 3-year-old classroom (discussion, story and song/movement activity); outdoor play; handwashing and restroom procedures; lunch; naptime and departure/pick-up of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. One previously cited violation (inspection date - 9/11/17) was reviewed for compliance; no repeat violations were cited. Refer to the accompanying Violation Notice for exceptions to compliance. Renovation/construction of the new classroom was discussed; please keep me informed of the progress as the new school year approaches. Standard #: 22VAC40-185-510-C Description: Based on observation of children's medications and discussion with staff on May 15, 2018, the program failed to ensure outdated medications were sent home and/or current medications were obtained. Evidence: 1. Review of child F's medication, on this date, determined the expiration date on the prescription medication was December 2017. 2. Review of child G's non-prescription medication, on this date, determined that the expiration date on the medication was February 2018. 3. Staff #1 confirmed the expiration dates on the children's medications. Plan of Correction: The outdated medications will be sent home. In the future, medications will be periodically checked to ensure children's medications are current and outdated medications are returned to parents.
Standard #: 22VAC40-185-520-D Description: Based on observation of children's topical skin products and discussion with staff on May 15, 2018, the program failed to maintain a record that included the child's name, date of use, frequency of application and any adverse reactions when insect repellent was used. Evidence: 1. Observation of topical ointments, on this date, determined that when child C's insect repellent was used , the application of the insect repellent was not documented. 2. Staff #2 confirmed the lack of documentation when insect repellent has been applied. Plan of Correction: A record will be maintained whenever insect repellent is used.
