An unannounced renewal inspection was conducted today, May 30, 2018 to determine the center's compliance with Licensing standards. The inspection was initiated at 3:00 pm and concluded at 5:30 pm. Upon the inspectors arrival school age children were working on making props for an upcoming play on the book, "The Wild Robot". Children created gardens with artificial flowers and mushrooms out of tissue paper. Children had created story boards and had been practicing their scripts. Later children were observed playing chess and researching information on dogs. All areas of the facility were inspected. The facility has age appropriate materials and supplies. Children provide their own meals and snacks. The first aid kit and non-medical emergency supplies were inspected and found complete. The following required information was posted: emergency number, the license, last inspection notice, evacuation map/procedures, and daily schedules. The inspectors interacted with staff and children in care. Staff records were reviewed in March 2018, no changes/ current. Board members records reviewed along with five children's records. Medication, authorizations and medication logs were reviewed (see violation notice). Staff have current CPR, First aid, daily health observation training and MAT certification. Today's Ratio Observed: 14 children with 3 staff. 12 middle school students with 1 staff. Violations cited at the previous inspection were checked for corrections. There was one repeat violation cited during today's inspection. A renewal application will need to be submitted to the Department before the expiration of the Conditional License. Standard #: 22VAC40-185-140-A Description: Based on review of children's records, the facility did not obtain documentation of a physical for a child before the child's attendance or within one month after attendance. Evidence: Child #3, enrolled 10/01/2016, did not have documentation of a physical on file. Plan of Correction: Per the Administrator: I will e-mail parents for most recent physical.
Standard #: 22VAC40-185-510-G Description: Based on review of medications, authorizations and documentation the medication administration log, the facility did not ensure medications administered to children were maintained in the original, labeled container. Evidence: 1. Documentation stated that the prescription medication for child #6 had been administered on 04/23/2018. 2. The prescription medications for child #6 did not have the original, labeled container. Plan of Correction: Per the Administrator: I will e-mail the parents to bring in the label and original container for the medication.
