22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 1:55am and departed at 2:15pm. At the time of the tour there were 28 children in care with 5 staff present. Children were observed during nap time. One child was observed being dropped off by a school bus. A sample of 4 children's records and 2 staff records were reviewed. The center did not have any medications.
Standard #: 22VAC40-185-60-A Description: Based on a record review the licensee did not ensure all required information was kept for each enrolled child. Evidence: Child #3 (dob 11/29/16) (doe 11/05/16) did not have the name, address or phone number of the child's father. Plan of Correction: The following corrective action was provided by the owner during the exit interview: The owner will contact Mom to obtain the father's information and will have her document it in the record.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: 1. There was rust on the floor in the boy's bathroom. When the inspector wiped the area with a tissue a brownish substance rubbed off onto the tissue. 2. There was a tall coat cubby on wheels in the 4 year-old class area. Although it had locks on the wheels it was easily able to tip. 3. The entrance to the playground had a wood border in front of the gate. There were two protruding nails from the top of the wood. Plan of Correction: The following corrective action was provided by the owner during the exit interview: The rust will be removed, the protruding nails will be removed and the coat cubby will either be thrown away or bolted to the wall.
Standard #: 22VAC40-185-280-B Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The storage closet was unlocked. It contained various cleaning products to include 2 bottles of bleach, 2 bottles of furniture cleaner and other cleaning solutions. Plan of Correction: The following corrective action was provided by the owner during the exit interview: It was locked during the inspection. The owner will remind staff to lock the door when finished in the closet.
Standard #: 22VAC40-185-550-C Description: Based on observation the center did not ensure procedures/maps were posted in a location conspicuous to staff and children. Evidence: The center had 2 buildings on the property. The infant building did not have Emergency evacuation and shelter-in-place procedures/maps posted in the building. Plan of Correction: The following corrective action was provided by the owner during the exit interview: The owner had taken it down due to renovations on the building. It will be posted today.
