Comments:
An unannounced complaint investigation was conducted with center director and center staff. Based on staff interviews, attendance records, and time cards, the allegations made by the complaintant regarding ratios were determined to be valid. Based on staff interviews and infant daily reports, the allegations made by the complaintant regarding diaper changing was determined to be valid. Complaint related: Yes
Description: Based on staff interviews and time card reports, the center failed to have one staff member for every four children, ages birth to the age of 16 months. Evidence: on 11/20/2017, one Infant room staff was alone with 6 infants from 7:00 a.m. to 7:12 a.m., on 11/21/2017, one Infant room staff was alone with 6 infants from 7:00 a.m. to 7:15 a.m., and on 11/22/2017, one Infant room staff was alone with 6 infants from 7:00. to 7:19 a.m.

Plan of Correction: One staff person was late for work several mornings and was counseled about the importance of arriving on time.

Complaint related: Yes
Description: Based on review of infant daily reports, the facility failed to document the immediate changing of diapers when wet or soiled. Evidence: on 11/22/2017, Infant 1 was checked in to the center at 7:32 a.m. and was picked up at 6:03 p.m. The only documented diapers for the day were 9:20 a.m., 11:54 p.m., and 2:43 p.m. When the parent picked up, the infant's diaper was heavily soiled and the child showed signs of diaper rash. On another infant report dated 11/27/2017, Infant 1 was checked into the facility at 7:44 a.m. and was picked up at 6:02 p.m. The only documented diapers for the day were 10:02 a.m. and 2:45 p.m. When the parent picked up, the infant's diaper was heavily soiled with urine.

Plan of Correction: Staff were counseled about changing diapers when soiled and checking more frequently to see if the diapers need to be changed.
