Standard #: 22VAC40-185-290-3 Description: Based on observation, unused electrical outlets did not have protective covers. Evidence: During the inspection, the two and three year old children were observed playing in the gym that had five unused electrical outlets that did not have protective covers. Plan of Correction: The gym belongs to the church and we use it when the weather is not good outside. I have asked the staff to check before the children go in there. I will have the staff take the bag of plugs with them to play in the gym and I can add it to my list of items to check in the morning.
Standard #: 22VAC40-185-500-B Description: Based on observation and interview, wet and soiled diapers cannot be disposed of in a way in which the caregivers do not need to use their hands in order to dispose of the diaper. Evidence: The foot petal for the foot operated trash can next to the changing table is broken, therefore the caregivers must use their hands to open the lid of the diaper pail. The program director confirmed that the teacher's had recently let her know that it was broken. Plan of Correction: We will buy a new one.
Standard #: 63.2-1720.1-B-3 Description: Based on record review, the center allowed staff to continue employment and supervise children without obtaining an out-of-state central registry (CPS) check from outside the Commonwealth of Virginia. Evidence: Staff #2's sworn disclosure statement confirmed that they have lived outside of the state of Virginia within the last five years. Center management did not submit the required out-of-state central registry background check for the state that the employee lived in. Plan of Correction: I have called our HR department to see if this has already been done. If it hasn't been completed we can work on getting it completed today.
