Background checks, medications, maintenance.
An unannounced renewal inspection was conducted from 10:15 am-2:00pm. 76 children were in care with 15 staff members present. Children were observed during circle time, leaving on field trip, doing water play, reading stories and doing craft activities. 5 staff and children records were reviewed. Medications, emergency drill records and supplies were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-130-B Description: Based on record review, center did not obtain additional immunizations every six months for children under the age of two. Evidence: Child # 1's record had immunization records from July 2017. There were no additional immunization records obtained by the center after that date. Plan of Correction: We will ask the parents to bring in updated immunization records.
Standard #: 22VAC40-185-70-A Description: Based on record review, a staff record did not include written information of the orientation training. Evidence: 1. Staff # 2 was hired on 8/31/17. 2. Staff # 2's record did not have written documentation to show that orientation training was completed within the first week of employment. Plan of Correction: We will complete the orientation with the staff person and place the paperwork in her file.
Standard #: 22VAC40-185-510-C Description: Based on record review, written medication authorization from the physician for long-term prescription medications had expired. Evidence: 1. The center had prescription and non-prescription long term medication for Child #2 on site. 2. The medication authorization for both medications had expired on 5/18/18. Plan of Correction: We will send the medications home today and ask them to bring in updated authorizations.
Standard #: 22VAC40-185-510-J Description: Based on observation, medications were not stored in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the two's classroom, prescription and non-prescription medication for Child # 2 was stored in a ziploc bag that was kept in a back pack hanging on a hook on a shelf. 2. The backpack was not locked, making the medication accessible to children. Plan of Correction: We will move the medication into a safe and locked container.
Standard #: 22VAC40-185-550-D Description: Based on record review and staff interview, center did not implement evacuation drills monthly. Evidence: 1. The fire drill record did not have any documentation to show that a fire drill was conducted for the month of June 2018. 2. Staff # 1 stated that the center did not conduct a fire drill for the month of June 2018. Plan of Correction: We will conduct two drills in July and record.
Standard #: 63.2-1720.1-B-3 Description: Based on record review and staff interview, center did not obtain an out of state central registry for staff that has resided out of the state of Virginia (VA) in the last five years. Evidence: 1. Staff # 1 was hired on 3/26/28. Her records indicated that she has resided outside of the state of VA in the last five years. 2. The results for the out of state registry for Staff # 1 have still not been obtained. 3. Staff # 2 was hired on 8/31/17. Staff # 2's records indicated that she lives in Washington D.C. 4. Staff # 1 stated that an out of state central registry for Staff # 2 has not been submitted or obtained. Plan of Correction: I will try to follow up for Staff # 1's central registry and if nothing is found I will resubmit. I will submit the out of state for Staff # 2 as soon as possible.
