Comments:
An unannounced renewal inspection was conducted from 9:30 am to 12:15 p.m. and 12:45 p.m. to 2:30 p.m. with the center director. There were forty-nine children in care ranging in age from four months to three years of age with ten staff. Infants were observed being fed, having playtime and sleeping. Older infants were observed during story time. Toddlers were observed completing puzzles and preparing for outside time. The two year old class was observed having imagination play, crafts, story time and during diapering. The three year old class was observed during rest time. The four year old class and the schoolagers were both on field trips to the pool. Classrooms were observed. The playground was inspected. Six children's records and six staff records were reviewed. Eleven medications and authorizations were reviewed. The last fire inspection was 1/24/2018. The last health inspection was 3/6/2018. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of documentation, the center failed to obtain documentation of immunizations before the first day of attendance. Evidence: Child B, enrolled 8/14/17, immunizations were dated 10/3/2017.

Plan of Correction: In the future, required documentation will be collected before the child's start date.

Standard #: 22VAC40-185-140-A
Description: Based on review of documentation, the center failed to obtain a physical exam within thirty days of attendance. Child B, enrolled 8/14/17, physical was dated 10/3/2017.

Plan of Correction: In the future, required documentation will be collected before the child's start date.

Standard #: 22VAC40-185-500-B
Description: Based on observation, diaper areas were not equipped with required items. Evidence: In the older infant room, the receptacle for soiled linens did not have a leakproof liner. In the Toddler and two Two year old classrooms, there was no leakproof receptacle for soiled linens.

Plan of Correction: The liner was placed during the inspection. The diaper areas will be provided with a soiled linen bin.

Standard #: 22VAC40-185-520-B
Description: Based on observation, sunscreen was not inaccessible to children under five years of age. Evidence: In the Two's classroom, the sunscreen was stored in the bottom of the unlocked changing table.

Plan of Correction: The changing table cabinet was locked during the inspection. Staff were reminded to move the sunscreen to a storage place out of reach of children.

Standard #: 22VAC40-185-520-C
Description: Based on observation not all diaper ointments were labeled with child's name. Evidence: In the infant room, one Desitin and one Extra Strength Desitin were not labeled with the child's name.

Plan of Correction: One ointment was disposed of during the inspection and the other ointment was labeled.

Standard #: 22VAC40-185-570-E
Description: Based on observation, not all bottles were dated. Evidence: Two Dr. Bown brand bottles for Child A were not labeled with the date.

Plan of Correction: The bottles were labeled with the date during the inspection. Staff were reminded to ensure all bottles are labeled with the date even if the parents forget to label the child's bottle.
