Comments:
An announced inspection was conducted to investigate a self reported incident that occurred on 6/25/2018 regarding staff supervision. Staff interviews, a review of the center's own video recording system, review of staff records, written staff statements, and observation of the senior summer camp classroom and playground area were conducted on this date. Standard #: 22VAC40-185-40-E
Description: Based on staff interviews and review of the center's own video recording system, the licensee failed to ensure compliance with the center's own policies and procedures. Evidence: l. The Center's Face to Name Policy requires that staff identify each child by face and name as each child crosses a threshold, staff are then required to change the child's location on an electronic device. On 6/25/18 ,The center's video recording system recorded Staff #1 failed to identify and enter the change of location of every child as the class was exiting the classroom to go outside to the playground. Child #1 was recorded leaving the end of the line and was left alone in the classroom. 2. The Licensing Inspector reviewed the electronic documentation completed by staff for documenting face to name for the date of 6/25/18. The majority of center staff failed to consistently document each change of location for each child of their assigned classroom.

Plan of Correction: A mandatory face to name training was held on July 11, 2018. All staff was trained all staff was trained except for one staff who was ill and one on vacation but both were trained by July 18th, 2018. Name to face will be done at every threshold by a different staff member who is not in single ratio. This is to be done so staff can eliminate the recurrence of any errors. The principal or assistant principal is to review the Tadpole transition report at least once weekly and coach staff as needed to ensure that consistent face to name implementation is done for all classrooms. Appropriate aggressive disciplinary action will be taken as needed to ensure that staff are doing name to face at every transition. Walkie talkies are to be used for all transitions in the building. If staff are in single ratio, administration is to be present for transition to observe name to face. The child that was left in the classroom was in a single ration situation. The implementation of management observation of single ratio transitions should prevent a reoccurrence of this type of situation. Administrators will document at least 10 face to name observations per week until further notice.

Description: Based on staff interviews and the center's video recording system, one child was not within actual sight and sound supervision of staff. Evidence: On 6/25/18, Child #1 was left alone in the senior summer camp classroom for approximately four minutes, while the the child's class transitioned outside to the playground. Staff #1 was unaware that the child was not with the remainder of the class until another staff found Child #1 alone in the classroom.
