Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted August 20, 2018. The on-site inspection was from 11:30a.m. to 2:35p.m. On this date, there were 18 children in four classrooms and in the care of five staff members. There was one additional staff member available to assist if needed. The total enrollment for the program was 37 children. There were five staff members' records and five children's records reviewed. There was no medication on-site or being administered, which is the policy of the provider. Four classrooms were observed. Five staff members and three children were interviewed. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. The center's theme for the week was Me and My School. Individual lunches were brought to school by the children and afternoon snack was provided by the center. Some items observed and reviewed during the inspection included: required posted information, first aid kit, emergency supplies, evacuation and lockdown drills, injury reports, over-the-counter diaper ointments and their parent authorizations, annual fire inspection, lunch, hand washing and bathroom procedures, diaper changing procedures, infant feeding and care, transitions, nap time and the outdoor play area. Standard #: 22VAC40-185-560-G
Description: Based on observation of five lunch boxes and verification from staff, five lunch boxes brought from home were not labeled per the requirements. Evidence: 1. The lunch boxes brought from home were checked for five children in Classroom A. Three of the five lunch boxes were not labeled with the children's names and all five of the lunch boxes were not labeled with the date.

Plan of Correction: The staff immediately labeled all lunch boxes during the inspection. Starting tomorrow, they have put a system in place to ensure all lunch boxes brought from home are labeled with names and dates.
