Comments:
The center self reported a situation involving forbidden action ( physically disciplining a child). Interviews and a review of written statements were conducted with the local department of social services. The self reported situation resulted in one violation. Please submit your plan of correction by 8/ 16/18. Arrival time: 11: 50 am Departure time: 2: 15 pm

Standard #: 22VAC40-185-410-1
Description: Based on interviews, it was determined that one staff engaged in a forbidden action when the staff physically punished a toddler. Evidence: On 6/29/18 at approximately 2: 45 pm, staff A "tapped" the leg of child A ( 20 months of age) in an effort to move the toddler's leg from the table.

Plan of Correction: Staff in the toddler room witnessed Staff A attempt to remove child's leg from the table by tapping the child's lower leg. Staff in the classroom recognized correction to be against policy, immediately responded and reported the incident to the Manager on duty. Staff A. was terminated.
