Comments:
An unannounced monitoring inspection was conducted on September 12, 2018. There were 14 children present with two staff members. The children were observed having breakfast which consisted of pancake and sausage on a stick, mixed fruit and milk. The children were also observed during toileting, handwashing, having circle time and playing in centers. The licensing inspector reviewed the physical plant, administration, required postings and documentation, health and safety supplies and procedures, nutrition and supervision. Two staff records were reviewed. Five children's records were reviewed. One medication was reviewed. The outdoor play area was not inspected due to inclement weather. Exit interview was conducted with the Program director/Lead Teacher.

Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on September 12, 2018, the fire inspection report for the facility was more than a year old and was dated 5/18/17.

Plan of Correction: We will continue to try to get in contact with the Fire Marshal.

Standard #: 22VAC40-185-290-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that fans, when used, shall be out of reach of children and cords shall be secured. Evidence: During the inspection conducted on September 12, 2018, the following was observed: 1. There was an oscillating fan on top of a set of wooden cubbies plugged into an extension cord that was hanging down to the ground and plugged into the outlet above the counter. 2. There was a tower fan and a small personal fan on a wooden desk with cords dangling and draped across a chair plugged into the outlet.

Plan of Correction: The fans will be moved to a safe place.
