A Subsidy Health and Safety inspection was conducted on 09/18/2018 to review supplemental health and safety requirements. The inspection began at 9:45am and ended at 2:45pm. There were 40 children in care with twelve staff. The children were observed at snacks, playing on the playground, during circle time and during lunch. Five staff?s records and five children?s records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: VENDSUB-000-004-F Description: Based on observation, interview, and record reviews the vendor failed to have posted or a document with a list of children's allergies which is immediately accessible to staff. Evidence: 1. The inspector observed the facility. There was no posted document or file with children's allergies. 2. The inspector asked the director if there was a document that listed children's allergies and the director said there was not. 3. The inspector reviewed five children's records. One of five records reviewed (child record #1) contained a diagnosed food allergy, but there was no posting or document available. Plan of Correction: The director will prepare a document to list all of the children's allergies, sensitivities and any dietary restrictions. This document will be placed in area that is immediately accessible to staff.
Standard #: VENDSUB-000-006-B-11 Description: Based on record reviews, the vendor failed to obtain a written allergy care plan for a child with a diagnosed food allergy. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) had a diagnosed food allergy; however, the record did not contain a written allergy care plan. Plan of Correction: The director will ask the parent to obtain and provide an allergy care plan for the child's record.
