22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:30 am - 11:30 am with the center director. There were 42 children, ranging from three years to five years of age, present with 6 staff supervising. Children were observed participating in a religion class, singing and dancing, having snack which is brought from home, and playing in the gym. Four children's records were reviewed and in compliance. Two staff records were reviewed. The emergency escape drill log was observed and in compliance. Standard #: 22VAC40-185-70-A Description: Based on review of two staff files, and an interview with the director, it was determined that the facility did not have all required information in each staff file. Evidence: For staff B (date of hire: 9/1/18) there was no documentation of job title, date of employment, orientation training or a second reference as to character and reputation as well as competency before employment. Plan of Correction: Second reference was checked during inspection. Other documentation will be completed this week.
Standard #: 22VAC40-185-240-D-1-b Description: Based on documentation review, and an interview with the director, the center did not have at least one staff member on duty who has obtained daily health observation instruction within the last three years. Evidence: There was no documentation that any of the staff members on site had current daily health observation instruction within the last three years. Plan of Correction: Will conduct the Daily Health Observation training at the next staff meeting.
Standard #: 22VAC40-185-260-A Description: Based on a review of documentation, the center did not have an annual fire inspection report. Evidence: The most recent fire inspection report on file was dated 5/30/17. Plan of Correction: Will contact station #10 and request a fire inspection.
Standard #: 22VAC40-185-260-B Description: Based on a review of documentation, the center did not have an annual health inspection. Evidence: The most recent health inspection report on file was dated 9/30/16. Plan of Correction: Will contact the health department and request a health inspection.
