Licensing inspector arrived at 8:40 am and departed at 12:00pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of arrival was thirty - five children in care with 7 staff . Additional staff arrived during the inspection. The Licensing Inspector observed children arriving , playing with blocks, diapering, bathroom break, and circle time. The sample size consisted of 5 children records and 8 staff records. Areas of non-compliance were reviewed with the director.
Standard #: 22VAC40-185-160-A Description: Based on 1 of 8 records reviewed, the licensee did not ensure staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on September 27, 2018, staff #1 ( employment date 09/10/2018) most recent tuberculosis screening was May 30, 2017 which was more than 12 months prior to employment . Staff #6 ( employment date 09/06/2018) most recent tuberculosis screening was July 3, 2017 which was more than 12 months prior to employment . Plan of Correction: 160-A: Staff have all been sent for an updated TB tests as of October 4, 2018. Management has been retrained on this criterion and DM will review all new hire TB tests for the next quarter to ensure compliance.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable. Evidence: During the inspection conducted on September 27, 2018, the following was observed : 1- In the bathroom next to the preschool area, the bathroom had a toilet lid cracked sitting on the toilet seat. The toilet lid could of been knocked over while students were using the restroom. 2- In both children's bathrooms , the water valves metal plates are rusting and a red color residue comes off on to the toilet pape when touched. 3- On the school age playground, there was trike with a cracked seat. Standard #: 22VAC40-185-280-B Description: Based on observation , the licensee did not ensure hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on September 27, 2018, the red cabinet in the two year old classroom was unlocked there was bleach and water spray and disinfectant spray . Plan of Correction: 280-B: Staff have been retrained as of 9/28/18 on proper chemical storage. Management will observe daily for compliance.
Standard #: 22VAC40-185-350-E-1 Description: Based on observation, attendance record review and staff interviews , the licensee did not ensure that for infants, there was one staff member for every four children. Evidence: During an inspection on September 27, 2018 , the licensing inspectors observed and reviewed head count forms in the infant classroom from ages 0-16 months. The center did not comply with the 1:4 ratio. In the infant classroom , two staff were with 9 infants from approximately 8:10am- 8:54 am. A third staff member arrived at 8:54 am. The infant classroom was not in ratio for forty -four minutes. Staff confirmed the 9th infant arrived at 8:10am. Plan of Correction: 350-E-1: Due to a change in child attendance, we have changed our infant schedule as of 9/28/2018 to ensure compliance. Staff have all been retrained to notify management immediately when they are at a ratio break, and additional children will put them over their current ratio allowance. Management will continue to hire to ensure that both the AD and CD don?t have to be attached to ratios when call outs occur.
Standard #: 22VAC40-185-500-B Description: Based on observation, the licensee did not ensure the diapering surface shall be used only for diapering or cleaning children Evidence: During the inspection conducted on September 27, 2018, the diapering surface in the toddler classroom, there was a cup, clipboard and artwork. Plan of Correction: 500-B: Staff have been retrained as of 10/2/18 on managing diapering surfaces. Management will observe daily for compliance.
Standard #: 22VAC40-185-550-C Description: Based on observation and interviews, the licensee did not ensure there was a map that includes shelter in place location posted in the facility. Evidence: During the inspection conducted on September 27, 2018, the shelter in place locations were not included on a posted map in the building. Confirmed with staff. Plan of Correction: 550-C: All updated maps with shelter in place locations have been updated and put up as of 9/28/18. DM will observe for this at all visits for the remainder of this fiscal year.
