An unannounced monitoring inspection was conducted from. to 11:50 a.m. to 2:15 p.m. At the time of entrance the following adult to child ratios were observed: Classroom D: 17 children, ages 3 to 4 years old, with 2 staff Classroom A: 18 children, ages 3 to 4 years old, with 2 staff Classroom B: 18 children, ages 3 to 4 years old, with 2 staff Classroom C: 17 children, ages 3 to 4 years old, with 2 staff The records sample size consisted of 7 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed eating lunch which consisted of Taco Salad (grouns turkey, lettuce, tomato, cheese, sour cream) Diced peaches, Spanish rice and milk. Hand washing, table sanitizing procedures, restroom procedures, and children settling on cots for nap period were also observed. Medication administration was not observed. However, 7 medications kept on site for emergency use were reviewed with regard to storage, written authorizations and required documentation. Thank you for your time during today's inspection. Standard #: 22VAC40-185-270-A Description: Based on observation, the program did not ensure that all areas and equipment of the center, inside and out, are maintained in clean, safe, operable condition. Evidence: 1. Two of the posts (blue) on the outdoor slide structure have peeling paint which is easily chipped away by brushing fingers across the surface. 2. The wheel bases / hubcaps on one of the two tricycles located on the outdoor play area has peeling yellow paint on them which is easily picked off in areas where it has come away from the metal. 3. Children have to potential to put pieces of chipped paint in their mouth or to get a "splinter" either from the raw rough wood on the posts or from the actual peeled paint on both the post and the tricycle. Plan of Correction: The contact staff (Staff #1) stated that she will submit a work request today to have these items sanded and painted. A call was placed during the inspection and it was arranged for the correct paint to be brought over and repairs made tomorrow.
