Technical Assistance:
Discussed with the Director: Member of the management team available at all times to classroom staff. Supervision of staff on an ongoing daily basis; the legal requirement for reporting any suspected child abuse or neglect to the proper authorities.

Comments:
An unannounced inspection was conducted on 10/15/18, 10/17/18 and 10/18/18 to investigate a complaint related to the supervison of children. Based on staff interviews, written statements and observation, it was determined that the complaint was valid. Description: Based on observation, it was determined that the front door is not operating properly. Evidence: The front door, through which the child left the building, is supposed to require two hands on the push bar to open. The Licensing inspector tested the door and determined that it took only one finger to push the bar and open the door..

Plan of Correction: The Director notified Corporate Office on 10/17/18 to have the door repaired and asked for a push exit button. Until it is repaired there will always be a staff person in the front lobby.

Complaint related: Yes
Description: Based on interviews with 3 staff and a review of the written statements of 1 staff and 1 parent witness, it was determined that the facility failed to provide care, protection and guidance as appropriate to the age of Child #1. Evidence: On Thursday, October 11, 2018, as a parent was leaving the Pre2 classroom at approximately 8:45 a.m., Child #1 ran out of the classroom when the door opened and ran down the hallway and into the lobby. He then went out the first door and then the second set of doors. The parent followed him and brought him back into the center. He was on the sidewalk in front of the center between the two handicapped parking spaces. At the time of the incident Staff A, had nine other children in her classroom, she called over the intercom for management assistance but no one was available or heard her call for assistance. The only management present in the building was the Director who was in the kitchen, having made breakfast and was now prepping lunch. Due to the exhaust fans in the kitchen, the call for assistance was not heard. Staff B did arrive at the front and took Child #1 back to his classroom. The parent of Child #1 was notified by the Director.

Plan of Correction: The following changes have been instituted: a Member of the management team will be now be at the front of the building and available to assist during morning arrival times and evening departure time; The Director will no longer be in the kitchen but rather will be immediately available to assist teachers as needed. A new Code Yellow procedure was instituted in which once the code is rrelayed over the intercom, all staff will open their doors to attempt to assist with the problem.
