63.2 Child Abuse and Neglect

An unannounced monitoring inspection was conducted on August 23, 2018 in response to a self-reported incident. The investigation was conducted jointly with the local department of social services and local law enforcement. The licensing inspector reviewed staffing and supervision, programs, administration, child abuse and neglect and background checks. Areas of noncompliance are identified on the Violation Notice.

Standard #: 63.2-1509-A
Description: Based on inspection of the facility and staff interviews, the licensee did not fulfill the responsibility to report certain injuries of suspected child abuse and/or neglect immediately to the local department of the county or city wherein the child resides or wherein the abuse or neglect is believed to have occurred or to the Department's toll-free child abuse and neglect hotline. Evidence: During the inspection conducted on August 23, 2018, it was determined that an incident allegedly occurred at the facility on August 10, 2018 where a four year old child presented with bruising on his upper left arm. 1. Staff #1 was made aware of the child's injury on August 13, 2018 but did not report the suspected abuse to child protective services until August 15, 2018. 2. When interviewed, Staff #1 indicated that she "dropped the ball" when it came to reporting the matter immediately to child protective services.

Plan of Correction: In future cases, the hotline will be utilized immediately.
