A Subsidy Health and Safety inspection was conducted on 11/01/2018 to review supplemental health and safety requirements. Present during the inspection were 46 children, ages 5 years-12 years, and 6 staff. The inspector reviewed 5 children's records and one medication during today's inspection. The staff records will be viewed on a later date at the Corporate Office. The children were observed in the following activities: during afternoon snack, during homework time, participating in free choice activities and in large motor play in the gym. The inspector reviewed the emergency procedures and emergency supplies during the inspection today. The inspector discussed the following with the site staff: attendance requirements and children's allergies. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety inspection were found and are identified on the Violation Notice. The inspector arrived for the inspection at 3:00 p.m. and departed at 5:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Evidence: The record for Child #1 had documentation of a diagnosed food allergy. The record for Child #1 did not contain a written allergy care plan. Staff #1 confirmed that there was not a written allergy care plan for Child #1.

Plan of Correction: The program director will get the from from the parent.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
