Comments:
An unannounced monitoring inspection was completed on 11/2/18. There were 54 children in four classes from two years of age through six years with eight staff directly supervising and two administrative staff present in addition. Six children's records and five staff records were reviewed. Five emergency medications were reviewed. The inspector arrived at the center at 10:00 am and departed at 12:45 pm. The children were observed during "work" time in the classes, during outside times, during story times, during lunch, and during preparation for nap.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all children's records contained documentation of all required information. Evidence: 1. The records of children #1, #2, #4 were missing the addresses for two emergency contacts for the children. Two emergency contacts with an address and phone number are required for each child.

Plan of Correction: The assistant director will contact the parents and have the addresses documented.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There was an unlocked cabinet observed under a sink in a classroom that had multiple cleaning supplies (spray of glass cleaner, two disinfectant sprays, container of disinfecting wipes, liquid soaps). The magnetic locking system on the cabinet door was not working properly and allowed the door to remain unlocked.

Plan of Correction: This was corrected during the inspection. The assistant director worked on the locking mechanism and got it working. The cabinet was locked after this.
