Technical Assistance:
Daily health screening training was discussed with administrators as the nurse is not in attendance daily.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:40 AM and departed at 12:40 PM. Fourteen children were in care. Morning activities, play in the gymnasium and lunch service were observed. A sample size of five staff records and five children's records were reviewed. Injury records were reviewed. Administration reported that there is no medication stored to be administered to children in care and that no medication has been administered to children in the licensed program this school year. The playground was not inspected due to inclement weather. An exit meeting was conducted prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-240-A
Description: Based upon review of five staff records, the facility has not ensured that each has received the required training prior to the first day of assuming job responsibilities. Evidence: 1. The record provided for staff 1, hired on 08/13/2018, did not include documentation that staff 1 received the required orientation training prior to the end of the first day of assuming responsibilities. 2. Administration verified that staff 1 did not attend the training day.

Plan of Correction: The facility responded with the following: Staff 1 will receive the required training which will be documented in her record.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: There were two containers of pump hand sanitizer on a table inside the door to the gymnasium. The containers are labeled "caution" and "keep out of reach of children". The preschool children were playing in the gymnasium.

Plan of Correction: The facility responded with the following: All staff will be reminded that potentially hazardous substances must be kept in locked locations when not in actual use.

Standard #: 22VAC40-185-550-M
Description: Based upon review of injury records, the facility has not ensured that a record of children's injuries in which all required information is entered the same day as the occurrence is maintained. Evidence: An injury record dated 8/24/2018 for child 1 did not include the time of the child's injury, the circumstance of the child's injury, future action to prevent recurrence of the injury and a staff and parent signature or two staff signatures.

Plan of Correction: The facility responded with the following: Injury records will be kept for all preschool and extended care children that include all required documentation.
