An unannounced renewal study was conducted today. There are 54 children after school with 5 staff. 5 staff records were reviewed for specific items only. The staff files are reviewed at another time at the main office. 8 children's records were reviewed. See violation notice for missing information. Injury reports, emergency supplies, first aid supplies, emergency escape drills and shelter-in-place drills are all in compliance. The playground was not inspected today due to inclement weather. Children were observed washing hands, eating snack, doing homework and playing with a variety of games in the room. A Subsidy Health and Safety Inspection was also conducted today.

Description: Based upon a review of medication authorizations, parental permission was not obtained to administer medication. Evidence: 3 of 3 medication authorizations reviewed did not contain documentation of the parent signature on the form. (Children 1,2,3)

Plan of Correction: I will have the parents sign the forms.
