Comments:
An unannounced monitoring inspection was conducted on 11/08/2018. Present during the inspection were 18 children, ages 2 months-4 years, 4 staff providing direct care and supervision and one support staff. The inspector reviewed 5 children?s records and five staff records during today?s inspection. The staff stated that medication is not administered. The children were observed in the following activities: having breakfast, free choice/center play, story time, going outside for a walk, and playing outside. Infants were observed being fed and allowed to sleep on demand. The inspector reviewed the emergency procedures and emergency supplies during the inspection today. The inspector discussed the following with the staff: subsidy requirements, background check requirements, nutrition and items from home. The inspector arrived for the inspection at 8:00 a.m. and departed at 11:15 a.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that the separate record for each enrolled child contained all of the elements as required by the standards. Evidence: 1. The record for Child #1 did not contain the following information: health information as required by 22VAC40-185-130 through 22VAC40-185-150, name of any additional programs or schools that the child is concurrently attending, documentation of viewing proof of the child's identity and age, and first date of attendance. 2. The record for Child #2 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached, the second contact had an incomplete address, and the name of any additional programs or schools that the child is concurrently attending.

Plan of Correction: The program staff will get the address and will get the other information from the other parent.

Standard #: 22VAC40-191-60-C-2
Description: Based on observations, record review and discussion with the staff, the facility failed to obtain a finding from a search of the central registry within 30 days of employment. Evidence: The personnel record for Staff #1 had a documented hire date of 09/04/2018. There was no record of search of the central registry for Staff #1. Staff #1 was observed working on 11/08/2018. Staff #5 stated that the Virginia State Police Criminal Record Check was sent rather than the search of the central registry.

Plan of Correction: The central registry search form was completed during the inspection and will be mailed tomorrow.
