63.2(17) License & Registration Procedures

Technical Assistance:
Timing: Sworn Statement or Affirmation - every five years Fingerprints - every five years Child Protective Services (CPS) Checks - every five years Out-of-state CPS Checks - upon hire Tuberculosis (Tb) screening - every two years

Comments:
Licensing Inspector arrived at 10:15 AM and departed at 12:30 PM to conduct a monitoring visit. At the time of the tour, there were 73 children (ages infant through preschool) in care with 18 staff. Children were observed participating in learning activities, preparing for lunch and eating. Medication, center documents and incident reports were reviewed. The sample size was five staff and six children's records. Violations associated with this inspection are noted on the attached violation notice.

Standard #: 22VAC40-185-130-B
Description: Based on review of six children's records, it was determined that the facility did not ensure that additional immunizations were obtained once every six months for children under the age of two years. Evidence: The following children lacked updated immunizations: - Child #2 (last immunizations 9/18/17) - Child #4 (last immunizations 8/20/18) - Child #5 (last immunizations 9/8/17)

Plan of Correction: Center management will request updated immunizations from parents. All files will be audited and immunizations will be updated as needed.

Standard #: 22VAC40-185-140-A
Description: Based on a review of six children's records, it was determined that the facility did not ensure, within 30 days of enrollment, each child had a completed physical examination on file. Evidence: The record for child #2 (date of enrollment 10/18/17, did not have documentation of a completed physical examination on file.

Plan of Correction: Center management will contact parent to obtain copy of physical examination.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of five staff records, it was determined that the facility did not ensure that all staff hired prior to obtained the implementation of fingerprinting no later that September 30, 2018. Evidence: Staff hired prior to implementation of fingerprinting had not completed fingerprinting.

Plan of Correction: Center management had misunderstood implementation procedure and began scheduling appointments during inspection.
