Comments:
An unannounced monitoring inspection was conducted on November 13, 2018 during the hours of 9:15 a.m. and 12:30 p.m. Thirty-four (34) children were enrolled and twenty-four (24) children were in the care of two/three staff members and two co-op parents, on this date. Two volunteers were also present. Five children's records, three staff records and two co-op parent records were reviewed and/or updated. Children and staff were observed during: free choice time (woodworking, sorting activity, imaginative play, making talking sticks, painting and sensory - removing corn from cobs); handwashing; snack; outdoor play; storytime and pick-up/departure. One previously cited violation (inspection date - 4/30/2018) was reviewed for compliance; no repeat violations were cited. Refer to the accompanying Violation Notice for one exception to compliance cited during this inspection. Thank you for your time and assistance. Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records and discussion with staff on November 13, 2018, the program failed to obtain documentation that one child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: 1. Review of child A's record, on this date, determined that an immunization record was not on file. 2. Staff #1 confirmed the lack of an immunization record in the child's record.

Plan of Correction: The child's parent will be notified and an immunization record will be obtained.
