An unannounced monitoring inspection was conducted on 11/14/18 from 3:00pm - 4:50pm. At the time of entrance there were 67 school age children in care with 10 staff. Children were observed participating in various activities in the cafeteria, playing outside, doing homework, and eating snack. Records were reviewed for five children and for eight staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-160-A Description: Based on a review of eight staff records, it was determined that the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employment starts. Evidence: 1. The record for staff #8 (date of hire 9/21/18) did not include documentation of a negative TB screening. 2. Staff #9 (Program Director) reviewed the record for staff #8, and confirmed that a negative TB screening has not been received. Plan of Correction: The facility responded: Staff #8 will be sent to complete a TB screening. All new staff will complete A TB screening within 21 days of employment.
Standard #: 22VAC40-185-550-D Description: Based on a review of the emergency drill log it was determined that the licensee did not ensure that the center shall implement a monthly practice evacuation drill. Evidence: 1. There was no emergency evacuation drill documented for September 2018. 2. Staff #9 (Program Director) confirmed that an emergency evacuation drills was not completed during the month of September 2019. Plan of Correction: The facility responded: We will ensure that we complete a monthly emergency evacuation drill and document it.
