63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted today from 7:05am-8:45am. There were 19 children (5-12yrs.) supervised by 3 direct care staff. The physical plant, 2 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing Legos, board games, and creating bead art. Hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 3 of 4 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 8/31/2016) record did not contain documentation of a completed physical form signed by a physician. 2. Child #3's (start date: 8/28/2017) record did not contain documentation of mom's work and work telephone number. 3. Child #4's (start date: 9/1/2016) record did not contain documentation of dad's employer. Plan of Correction: I will get all missing information today.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the main classroom, a cabinet underneath the sink contained various cleaning supplies and chemical solutions. The cabinet had a bike lock that allowed the cabinet doors to be opened and making the cleaning supplies accessible to children. 2. In the second classroom, an unlocked cabinet underneath the sink contained a bottle of Lysol cleaning spray and WD-40. Plan of Correction: All supplies were moved to a locked cabinet during the inspection.
Standard #: 22VAC40-185-510-N Description: Based on a review of medications, 1 of 5 medications was not picked up within 14 days of the authorization expiration, and the medication authorization was not renewed. Evidence: Child #4's (start date: 9/1/2016) Children's Pain Relief had a corresponding medication authorization form that was dated 11/1/2018 and was not signed by a physician (expired: 11/8/2018). Plan of Correction: The medication will be returned to mom during pick-up this afternoon.
