- Technical Assistance:
Consultation was given on annual training requirements, hand washing procedures, and emergency preparation.
An unannounced renewal study was conducted today from 10:00am-12:30pm. 16 children were present in 4 classrooms with direct care staff. Children were observed in center activities, crafts, reading, free choice, and infant care. No emergency medications were reported on site today. A random sample of children and staff files were reviewed. Emergency preparation was current. Rooms are well equipped and maintained. Lunch to be served today was tacos, fruit, rice and milk. Standard #: 22VAC40-185-40-J Description: Based on center management statement, an update of injury prevention procedures was not documented. Evidence: 1. Management stated that no documentation was maintained when reviewing injuries Plan of Correction: Moving forward, injuries will be reviewed and documentation will be maintained at the end of every school year.
Standard #: 22VAC40-185-60-A Description: Based on record review, 2 of 5 children's records did not contain a date and signature that the proof of ID was reviewed. Evidence: 1. Child #4 and Child #5's record did not contain the individual's signature who reviewed the proof of ID and the day it was completed. Plan of Correction: Staff member updated forms during inspection.
Standard #: 22VAC40-185-210-A-4-b Description: Based on record review, 2 of 3 staff did not complete the required 16 hours of annual training. Evidence: 1. Staff #1 and Staff #2 have completed 11.5 hours of training the past licensure year. Plan of Correction: Moving forward all staff will complete the required 16 hours of annual training.
Standard #: 22VAC40-185-520-C Description: Based on observation, 1 diaper ointment was not labeled with the child's name. Evidence: 1. 1 diaper ointment in the infant room was not labeled with the child's name. Plan of Correction: Ointment was labeled with the child's name during inspection.
