Technical Assistance:
Use of the water filled infant bottle warmer was discussed with the program director and infant staff.

Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 11:15 AM and departed at 2:40 PM. Nineteen children were in care with four staff. A sample size of five children's records and five staff records were reviewed. Lunch service, nap time and after nap activities were observed. Injury records were reviewed. It was reported that no medication is administered to children in care. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-570-C
Description: Based upon review of records, the facility has not ensured that for each child on formula, the record contains the feeding schedule. Evidence: 1. The record for child 3, the only infant currently enrolled who drinks formula, did not include the feeding schedule. 2. Staff 1 confirmed that there was not a record of child 3's feeding schedule.

Plan of Correction: The facility responded with the following: The infant's feeding schedule will be obtained from the parent today. A form will be developed to be completed by parents upon enrollment and updated as necessary.

Standard #: 22VAC40-185-570-E
Description: Based upon observation, the facility has not ensured that prepared infant formula is dated. Evidence: 1. There were two prepared bottles of formula for child 3 in the refrigerator in the infant classroom. Neither of the bottles were dated. 2. Staff 5 verified that the two prepared bottles for child 3 were not dated.

Plan of Correction: The facility responded with the following: The teachers will be instructed to ensure that all prepared bottles are dated upon arrival of each infant.
