Technical Assistance:
Discussed ways to streamline all record keeping to help achieve compliance with the Minimum Standards. There shall be a back-up plan for outdoor play in the event of snow/ice covered conditions. The emergency plan shall include clear wording for the sounding of alarms for each emergency scenario.

An unannounced monitoring inspection was conducted today. There were 7 children in care, with a total of 5 staff assigned to their direct supervision. Children's records, staff training records, background checks, health records, administrative records, the physical plant, classroom programming, and emergency preparedness items were inspected today. The children were observed playing with toys, eating snack and lunch from home, and interacting with caregivers. Patient, calm, and friendly interactions were observed with all children and all caregivers. This inspection was completed between approximately 10:20am and 12:30pm. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record A does not include: each parent's home phone number and work phone number; documentation showing that a staff person has viewed the child's original proof of identification. 2. Child record B does not include two emergency contacts that are not the child's parents. 3. Child record E does not include each parent's home phone number and work phone number.

Plan of Correction: All parents will be asked for the missing information.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training records, it was determined that two records do not include documentation of fully completed orientation training requirements. Evidence: Staff record F does not include: completed/signed/dated documentation showing the completion of First Day Orientation Training; documentation of completed "Recognizing Child Abuse and Neglect" training. Staff record G does not include: the trainer's signature/date on the First Day Orientation Training documentation; documentation of completed "Recognizing Child Abuse and Neglect" training.

Plan of Correction: Both of them completed orientation training. The documentation will be completed and placed in each file. I will make sure both of them complete "Abuse and Neglect" training this week.

Standard #: 22VAC40-185-560-G
Description: Based on observations, it was determined that two children's lunch/snack bags from home were not labeled as required. Evidence: Two children's lunch/snack bags were not labeled with each child's name and current date.

Plan of Correction: We will begin using a new and better system for labeling the children's food bags.

Standard #: 22VAC40-191-50-A-1-a
Description: Based on a review of staff background checks, it was determined that two new staff members do not have a fully completed Sworn Disclosure Statement. Evidence: The signed Sworn Disclosure Statement for staff F and G do no not include the question asking whether or not the individual has resided outside of the state of VA within the past 5 years.

Plan of Correction: I will have each of them sign the correct form.
