An unannounced monitoring inspection was conducted today from 11:30am-2:30pm. There were 72 children (infants-6yrs.) supervised by 12 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed having yoga class, reading a story, eating lunch and napping. Lunch today was chicken, rice, bananas, carrots, cheese and milk. Hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 4 of 5 children's records did not contain documentation of all of the required information. Evidence: 1. Child #1's (start date: not documented) record did not contain documentation of either parent's place of employment and work phone number, either of the two required emergency contact persons, persons authorized to pick up, first day of attendance, previous schools attended, annual update of emergency contact information, written agreements between the parent and the center as required by 22 VAC40-185-90, and proof of identification. 2. Child #3's (start date: 8/27/2018) record did not contain documentation of a complete address for the second emergency contact person and proof of identification. 3. Child #4's (start date: 4/20/2018) record did not contain documentation of proof of identification. 5. Child #5's (start date: 8/27/2018) record did not contain documentation of a physical form completed by a physician. Plan of Correction: I have already sent home missing to documents to parents.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center were not maintained in a clean, safe and operable condition. Evidence: In the The bottom wall and door frame of the back door was rusted and had chipped and peeling paint. Plan of Correction: I will reach out to the church for repairs.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Primary 1 (ages 3-6 years) classroom, a cabinet contained a bottle of Clorox bleach and the child safety lock was broken. 2. In the Infant (ages 6 weeks-12 months) classroom, a cabinet contained a container of Clorox wipes and the child safety lock was broken. Plan of Correction: I will reach out to the church for repairs.
