The unannounced complaint inspection was completed on 01/31/2019 from 12:30 P.M.- 1:15 P.M. The inspection was in response to a complaint that was received in the Valley Licensing Office on 01/11/201. The complaint was in regards to the licensing standards relating to operational responsibilities. There were 39 children present in the care of nine staff members and one administrator. Interviews with staff, and observation of the facility took place. The information gathered during the investigation does support the allegation of staff qualifications and is determined to be "valid". There exception to compliance are note on the violation notice. Description: Based on observation, the center failed to display the most current inspection report and license in a place conspicuous to the public. Evidence: 1. The licensing inspector observed that the most current license and inspection report dated 11/13/2018 were hanging in the entrance area behind the reception desk, and was not accessible to the parents or the public. 2. Staff verified that the license and inspection report were posted in the area behind the reception desk.

Plan of Correction: The program director immediately moved the license and the inspection report to a parent board in the hallway that is accessible to the parents and the public.
