Please forward the renewal application prior to the expiration date of your license. Lead qualifications were discussed.
An unannounced Renewal inspection was conducted this date. A total of 87 children with 12 staff present during the inspection. Observation of all 8 classrooms was conducted. Staff interviews, observation of programming, lunch and transition to nap time was made. A total of 6 children's files and 5 staff files were reviewed, all medications and required documentation were also reviewed. This inspection began at 10:10am and ended at 3:15pm. Standard #: 22VAC40-185-70-A Description: Based on review of staff records, two records were found incomplete. Evidence: Staff #1 and Staff #5's records did not include documentation that staff were qualified for a lead teacher position. Plan of Correction: Administrator requested proper documentation from staff #1 and #5. The documentation must be provided by Feb. 28, 2019.
Standard #: 22VAC40-185-270-A Description: Based on observation of the physical plant, areas of the building was not maintained in a clean and safe condition. Evidence: The blind in the Intermediate Class was torn at the bottom and part of the plastic was exposed. The green rug in the Intermediate class was found soiled. The base molding in the Infant 2 class near the book shelf was torn. The base molding located in the Intermediate Class near the front window is no longer attached to the wall in one area. Plan of Correction: The broken blinds in the Intermediate classroom have been removed. The green rug has been removed to be steam cleaned. If the condition does not improve, it will be replaced. Administration has contacted maintenance to fix the base molding in the Intermediate and Infant 2 classrooms. Repairs should be completed by the end of February.
Standard #: 22VAC40-185-280-B Description: Based on observation of the Intermediate Class, hazardous substances were found unlocked. Evidence: An unlocked cabinet in the Intermediate class contained a bleach spray. Plan of Correction: When inspector brought this to the teacher?s attention, teacher immediately locked the cabinet. Moving forward, teachers will be utilizing a licensing checklist to ensure their classrooms are up to code. Checklists will be turned in weekly, signed by a teacher and dated.
Standard #: 22VAC40-185-500-A Description: Based on observation of the Infant 2 class, hand washing procedures were not followed. Evidence: Licensing Inspector observed staff seating children for lunch without ensuring their hands were washed. Plan of Correction: Teacher immediately washed the childrens? hands when reminded. Hand washing procedures will be reviewed in the February staff meeting.
Standard #: 22VAC40-185-500-B Description: Based on observation of the Toddler and Beginner classrooms, diapering procedures were not followed. Evidence: The diaper surface in the beginner class has a crack and is no longer absorbent. The diaper surface in the Toddler class was not cleaned and sanitized adequately. Debris and glitter was found on, around and under the diaper surface. An Ipad was found on the diaper surface in the Toddler class. Plan of Correction: The diaper surface in the Beginner class is being replaced. In our February staff meeting, we will discuss the importance of cleanliness in the diapering area to avoid cross contamination. The diaper surface in the toddler class was cleaned today.
Standard #: 22VAC40-185-520-A Description: Based on review of over the counter skin products, two products were kept beyond the expiration date of the product. Evidence: In the Toddler Class, Aquaphor was found with an expiration date of March 2018 and Desitin was found with an expiration date of September 2018. Plan of Correction: When ointments, creams, or other topicals are brought in by parents, staff will label them with the child?s first and last name, and the expiration date. Staff will continue to monitor the expiration dates.
Standard #: 22VAC40-185-520-C Description: Based on observation of the Intermediate classroom, topical ointments were accessible to children. Evidence: Three topical ointments were found in 3 child cubbies. This area is accessible to children. Plan of Correction: Topicals were either sent home or locked away in a centralized location where children will not be able to access them.
Standard #: 22VAC40-185-550-M Description: Based on observation of the Infant classroom, an incident report was incomplete. Evidence: An incident report dated 1/17/19 was not signed by two staff or one staff and a parent. Plan of Correction: The incident reports were removed from the class today. During the February staff meeting, the importance of proper documentation and completion of incident reports will be reviewed. The expectation is for Incident Reports to be brought to the office for review and filing daily.
Standard #: 22VAC40-185-570-E Description: Based on observation of the Infant 2 class, a baby bottle containing infant formula was not properly labeled. Evidence: One baby bottle located in the refrigerator was not dated. Plan of Correction: At the staff meeting in February, the importance of proper bottle labeling will be discussed. Additionally, we will review bottle labeling procedures with parents.
