63.2 Facilities & Programs.
An unannounced renewal inspection was conducted on 2/20/19 from 11:00am until 1:45pm. At the time of the tour, there were 44 children in care with 7 staff present. A sample of 5 children's records, 5 staff records and 5 board member records were reviewed. Children were observed participating in morning learning activities, eating lunch and resting quietly during nap time. Hand washing, restroom and food service procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and center postings were reviewed. Two violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the center did not ensure that staff members shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: 1. The most recent tuberculosis test for staff 1 is dated 12/21/16. An updated test was due in December 2018. 2. The center director confirmed that the tuberculosis test located in staff 1's record is expired. Plan of Correction: Staff 1 stated that she obtained an updated TB test in 2017 and will contact her physician to obtain a copy, which will be placed in staff 1's record.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center did not ensure that each child's record contains all the required information. Evidence: 1. The records for child 1 and child 2 do not contain the addresses of two designated people to call in an emergency if a parent cannot be reached. 2. The center director confirmed that the records for child 1 and child 2 do not contain the addresses of the two required emergency contacts. Plan of Correction: The center director will request that the parents of child 1 and child 2 provide the addresses for the required emergency contacts, which will be placed in the children's records.
