Comments:
An unannounced renewal inspection was conducted on February 19, 2019, between the hours of approximately 10:05 a.m. and 1:55 p.m. Forty-three (43) children were in the care of six staff members during the on site inspection. Four additional staff members were present and assisted, as needed. Five children's records were reviewed and five staff records were reviewed and/or updated. Three children's medications and authorization forms were reviewed. Program activities, daily routines, interactions with children, staffing, supervision, equipment, posted information, first aid/emergency supplies, emergency evacuation/shelter-in-place drill records, snack/menu, classrooms and the outdoor playground areas were observed/reviewed. Lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage program specific learning and play materials. Program observation was completed. Activities observed on this date included: circle time in the 3 through 6-year-old classroom (story and discussion); outdoor play; handwashing and restroom procedures; lunch; naptime and departure/pick-up of children. Staff members were observed providing attentive supervision during activities and transitions. Previously cited violations (inspection date - 11/29/2018) were reviewed for compliance; no repeat violations were cited. Substantial compliance with the licensing standards was maintained; exceptions to compliance were noted on the accompanying violation notice. Options regarding mixed age grouping of children during outdoor play were discussed. Thank you for your time and assistance. Standard #: 22VAC40-191-40-D-1-A
Description: Based on review of background clearances for three board officers and discussion with staff on February 19, 2019, the program failed to obtain a sworn statement and a central registry clearance for one board officer within 30 days of service

Evidence:
1. Review of board officer CC's record, on this date, determined that the sworn statement and central registry clearance were obtained more than 30 days after the initial date of service (date of service - 10/16/2018, date of sworn statement - 2/19/2019 and date of central registry clearance - 1/03/2019).
2.. Staff #1 confirmed the dates of service, the sworn statement and the central registry clearance.

Plan of Correction: In the future, sworn statements and central registry clearances will be obtained in a timely manner for board officers.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of three board officers background clearances and discussion with staff on February 19, 2019, the program failed to obtain fingerprint results for two board officers in a timely manner.

Evidence:
1. Review of background clearances for board officers BB and CC, on this date, determined that fingerprint results were obtained on 12/19/2018. Board member BB's initial date of service preceded September 30, 2018. Board member CC's initial date of service was 10/16/2018.
2. Staff #1 confirmed the dates of service and fingerprint results.

Plan of Correction: In the future, fingerprint results for board officers will be obtained in a timely manner.
