Technical Assistance:
Licensing inspector discussed requirements for allergy lists.

Comments:
A Subsidy Health and Safety inspection was conducted on 02/27/2019 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 7am-9:30am. There were 23 children in the direct care and supervision of 8 staff. A sample of 5 children?s records and 5 staff records were reviewed. The children were observed engaged with staff, putting together puzzles, being dropped off by parents, and eating an early snack. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the administrator. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review and interview with staff, lockdown procedures were not practiced at least annually. Evidence: There was no record of lockdown procedures conducted for 2018. Staff A stated that she was unaware of the requirement.

Plan of Correction: I will add a time and date to practice lockdown procedures. One will be conducted today.
