Comments:
A Subsidy Health and Safety inspection was conducted on March 1, 2019 to review supplemental health and safety requirements. There were 23 children and 4 staff present. The ages of the children ranged form 4 to 11 years. The children were observed in the cafeteria and in the gym. Staff were engaged with the children. The children were served an afternoon snack. The children were in two groups and each group played games in the gym and completed an art activity in the school cafeteria. The children were served an afternoon snack. A sample of 6 children's and 4 staff records was reviewed. Documentation of the annual fire and health inspections was current. There were no medications in the center. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations found during the supplemental subsidy health and safety inspection are identified on the violation notice. Thank you for your assistance during this inspection.

Description: Based on a review of staff records, the center failed to obtain documentation of a TB screening completed within the last 30 days of the date of employment and prior to coming into contact with children. Evidence: The record of staff 3 did not contain documentation of a current TB screening. Staff 3 was working with the children at the time of the inspection.

Plan of Correction: The Center will ensure documentation of a screening is obtained and on file prior to the staff member working in the program with the children
