Comments:
An unannounced monitoring inspection was conducted today from 9:00am until 12:00pm. There were 97 children in the care of 13 staff members. The children were observed playing outdoors, doing yoga, listening to stories and playing St. Patrick's Day games. Stand and children's records were reviewed. There were no medications on site. Emergency drill records, emergency supplies, accident logs and policies and menu's were also reviewed. A complete walk through of the center and playground was also conducted. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of four children's records, the center failed to keep current records for each child enrolled. Evidence: 1) Child #1's proof of identification has no signature. 2) Child #2's file has no proof of identification.

Plan of Correction: Will get from parents and will add to child's file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that the outside shall be maintained in a clean, safe and operable condition. Unsafe conditions include chipped or peeling paint. Evidence: The building on the playground has peeling and chipping paint.

Plan of Correction: We will talk to the church about repairing it.

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient surfacing did not comply with minimum safety standards where playground equipment is provided. Evidence: Mulch at bottom of slides on playground is less than an inch in thickness (ground is visible).

Plan of Correction: We will add more mulch and rake the compacted mulch.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center failed to ensure that a minimum of two shelter-in-place practice drills were completed annually. Evidence: The center had documentation of only on shelter-in-place practice drill in October 2018.

Plan of Correction: Not available online. Contact Inspector for more information.
