Technical Assistance:
Floor tiles in the hall bathrooms need to be replaced as they are starting to deteriorate and smell. Additional outside play equipment such as push toys, cars and trucks need to be purchased when more than one classroom is on the playground at one time. Consultation was given on the enforcement action taken by VA DSS following the self reported incident in October. A letter informing the center of the Department's action was mailed on 3/1/2019 to the Corporate Headquarters. The Notice of Intent letter will need to be posted at the main entrance as required by General Procedures.

An unannounced renewal study was conducted today. All rooms were in ratio with direct care staff. Observed infant care, diapering, and singing, outside play time, toddle play on the indoor slide and reviewing colors, transitions to hand washing and preparation for lunch. Lunch served today was glazed chicken. brown rice, broccoli mandarin oranges and milk. Emergency medication is on-site. Reviewed 6 staff records, annual, Daily Health Observation, FA/CPR certification and Medication Administration Training (MAT). Injury plans, drills and medication authorizations were reviewed. Description: Based on record review, 1 of 7 children's records is incomplete. Evidence: Child #4's record did not contain two complete emergency contact addresses.

Plan of Correction: Center management will contact the parent to complete the missing information.

Standard #: 22VAC40-185-290-3
Description: Based on observation, an outlet in the Discovery classroom was missing a cover, Evidence: One unused outlet in the classroom under the cabinets by the radio was missing a protective cover during the inspection,

Plan of Correction: The outlet cover was replaced during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a foot operated trash can was not in use by the diapering changing table in the toddler classroom by the outside door. Evidence: The diaper disposal trash can was not foot operated and required staff to use their hand to open the container.

Plan of Correction: A new trash can that does not require a hand to operate it will be purchased.

Description: Based on observation, emergency phone numbers was not posted in a visible place by each telephone. Evidence: A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center was not posted in a visible place by the phone in the Discovery classroom.

Plan of Correction: A chart with emergency numbers will be posted in the Discovery class by the phone.

Description: Based on paperwork review, 3 of 6 accident reports was incomplete. Evidence: 3 reports did not contain the information on how the parent was notified and the time of the notification.

Plan of Correction: Staff will be reminded and retrained to complete all lines on the accident report form.
