An unannounced full renewal inspection was conducted today. There were 6 children in care, with a total of 4 staff members assigned to their direct supervision. Children's records, staff records, staff training, background checks, the physical plant, emergency preparedness items, children's medications, and classroom programming were inspected today. The children were observed eating snack from home, playing with toys, engaging in creative play, playing outside with chalk, reading books, playing games, and engaging in skill-specific interactions with staff. This inspection was completed between approximately 10:15am and 11:50am. Thank you for meeting with me today. Standard #: 22VAC40-185-280-B Description: Based on observations, it was determined that one potentially hazardous substance was not kept in a locked manner. Evidence: In the children's bathroom, one spray bottle of air freshener was stored on an open cabinet shelf. Plan of Correction: The bottle will be moved to the locking cabinet.
Standard #: 22VAC40-185-280-H Description: Based on observations, it was determined that two staff purses were not stored in a manner to keep them inaccessible to the children. Evidence: The Inspector observed two staff bags of personal belongings stored beneath a table in the children's classroom area. Plan of Correction: All staff will be reminded to store their bags in the office.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that one form was not correctly completed to authorize school staff to administer the medication. Evidence: One child's long term medication authorization form does not include the correct name of the school, per parent's authorization to administer the medication. Plan of Correction: The parent will be asked to update the form and include the correct school name.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medication authorization forms, it was determined that one authorization does not include full dosage instructions. Evidence: One child's long term prescription medication authorization does not include the physician's full dosage instructions. Plan of Correction: The parent will be asked to bring the physician's dosage instructions order.
Standard #: 22VAC40-185-510-G Description: Based on a review of children's medications, it was determined that one medication was on site without the required labeling. Evidence: One child's long term prescription medication was on site without the prescription label. This medication was also not in the original prescription packaging. Plan of Correction: The parent will be asked to supply the medication in the prescription medication bottle.
