A Subsidy Health and Safety inspection was conducted on March 19, 2019 to review supplemental health and safety requirements. Children were observed playing on the playground, cutting out astronauts, having circle, painting ice cream cones and snowmen, and practicing letter/sound recognition. Reviewed 8 staff files and 2 children's files, fire and health inspections, emergency preparedness drill log, medications and authorizations, playground and physical plant. 2. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-580-B Description: Based on record review, staff did not complete the Virginia Preservice Training for Child Care Staff within the first 90 days of employment. Evidence: Staff#1 and Staff#2 did not have documentation of Preservice training. Plan of Correction: Staff will have it completed in a week.
Standard #: 22VAC40-665-580-K Description: Based on staff record review, there is no one on site who has obtained within the last three years instruction in performing a daily health observation of children. Evidence: Staff were last trained on 7/28/2014. Director has a DHO training schedule on3/27/2019 Plan of Correction: I will scan a copies of the certificates.
