Comments:
An unannounced monitoring inspection was completed on 03/21/2019. Present during the inspection were 13 children, ages 3 years-5 years, in the direct care and supervision of two staff members. There was an additional support staff member on-site during the inspection. The inspector reviewed 5 children's records and the on-site staff record requirements for three staff members during the inspection. The staff stated that there is no medication being administered at this time. The children were observed in the following activities: circle time/morning greetings, music and movement, having a morning snack, brushing teeth, free choice/center play, and in one-on-one learning activities. The inspector reviewed the emergency evacuation plan, the shelter-in-place plan and the emergency supplies during the inspection. The inspector discussed morning meal routines with the staff. Standard #: 22VAC40-185-280-G
Description: Based on observations and discussion with the staff, the facility failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents. Evidence: Staff #3 was observed with a clear plastic spray bottle; the bottle had previously been labeled to indicate the contents but the label was no longer legible and did not clearly indicate the contents of the container. Staff #3 stated that the bottle contained a bleach and water solution.

Plan of Correction: The container was clearly labeled during the inspection.
